In preparation for the 2021-2022 school year and in compliance with the Department of Health, we are asking every parent to comply with the following requirements as part of the medical records for each child (PPK-12). Please note that some forms must be original, while the other ones can be photocopied and uploaded into our system.

Parents must sign in to MySJS, click on the child's name and look for the CONTACT CARD tab and update the emergency contacts. Parents then must go to the MEDICAL tab and scroll down to find all the medical forms that must be submitted electronically. This process must be completed on or before August 6, 2021.  

These original documents are mandatory, and must be submitted to the School Nurse's Office:
  • The Medical Examination Form must be downloaded and completed by the student's physician and student's parents.
  • Dental Certificate (received from your dentist's office)
  • PVAC-3 Form (Green Immunization Formmust be current and resubmitted as booster vaccines are administered. All children at ages 4, 11, and 16 require vaccines. Please speak with your pediatrician.
Please enclose the original forms above in a sealed envelope with the child's name and grade included. These documents may be dropped off with Security at Nairn Street any workday up to and including August 6, 2021 from 8:00 a.m. to 2:00 p.m. 

Every student must present a completed Covid-19 Test Administration Consent Form as required by the P.R. Department of Health. Please print out this form, fill it out and return to the Nurse's Office in a sealed envelope by the first day of school.

  • Student's Emergency Contact Form (mandatory)
  • Medicine Administration Consent Form (mandatory)
  • Copy of Covid-19 Vaccination Card (mandatory for all students age 12 and above.) Please choose "Decline" if not applicable.
  • Proof of Influenza Vaccination (highly recommended) The ideal vaccine time is Sept/Oct. Please upload when available; otherwise please choose "Decline."
  • Self-Carry Medication Form for 7th-12th grade students (when applicable.) Those students needing a self-carry form for a chronic medical condition (such as: Epi-pen, diabetes, etc.) must visit the School Nurse's Office on the first day of school. Please choose "Decline" if not applicable.

Sunny Semidei, RN BSN is a registered nurse who attended Mercer Medical Center School of Nursing and Thomas Edison State University. She holds nine graduate credits towards an MSN and is licensed in several states.

In 2017and 2019, Ms. Semidei traveled to Zambia to volunteer as the pharmacy lead in a project that served around 5,500 residents living in poverty and with numerous medical concerns. In addition, Ms. Semidei has sent over 100 pairs of used eyeglasses, donated by the SJS community, to various communities in Zambia through OMNI (Orphan Medical Network International.)

Ms. Semidei recently completed the Mindfulness-Based Stress Reduction (MBSR) program. She believes that health is a life-style and preventive care is paramount. She looks forward to being a part of initiatives that foster communication, education, and current research to promote wellness in our community that will positively impact our population for years to come.

Sunny Semidei is a leadership succession board member in the Sigma Theta Tau International 2021, Upsilon Rho Chapter. She was awarded the SJS Head of School's Service Award for the 2019-2020 school year.

Saint John’s School

Saint John’s School is a non-profit, college preparatory, nonsectarian, coeducational day school founded in 1915. The school, located in a residential area of Condado, in San Juan, Puerto Rico, has an enrollment of over 800 students from pre pre-kinder to grade twelve. Instruction is mostly in English with the exception of language courses.
