In preparation for the school re-opening in March 2021 and the 2021-2022 academic year, we are asking every parent to complete the following required forms for each of your children.  Please login to your MySJS account and complete all forms before August 1, 2021.
The Medical Examination Form must be downloaded and completed by the student's physician and student's parents, and handed in to the School Nurse, before the first day of school in August 2020 for admission.

PVAC-3 Form (Green Immunization Form)

New students - The Health Department of Puerto Rico requires that each child have an up-to-date original PVAC-3 on filie.

Returning students - A new original PVAC-3 form is required if any booster shots have been administered during the past year or during the current academic school year. Please confirm with your child's pediatrician, that all immunizations/boosters, such as Tdap and MCV4, are up to date.

Saint John’s School is a non-profit, college preparatory, nonsectarian, coeducational day school founded in 1915. The school, located in a residential area of Condado, in San Juan, Puerto Rico, has an enrollment of over 800 students from pre pre-kinder to grade twelve. Instruction is mostly in English with the exception of language courses.
